Authorizes Shore Community Services Inc. to release or obtain confidential records on behalf of an individual. Required for coordination with case managers, physicians, schools, and other providers. Compliant with HIPAA and Maryland Health-General Article §4-301.
The first step toward receiving services from Shore CSI. Collects essential information about the individual, their support needs, current living situation, and the services they are interested in. A team member will follow up within 5 business days of receipt.
Captures emergency contacts, physician information, current medications, insurance details, and known allergies. This form is kept on file at each residential site and is reviewed regularly to ensure accuracy.
Helps our team truly get to know the individual we will be supporting — their interests, goals, daily routines, communication style, strengths, and what a good day looks like for them. This form shapes how we build each individual's support plan.
Use this form to report a concern, incident, or complaint regarding services. All reports are reviewed within 2 business days. For emergencies, call 911 first, then notify our office. Completed forms may be submitted by email, fax, or mail.
